Are you registered with a GP?

To get the most from the NHS, you must be registered with a GP practice.

If you are new to Hounslow, it is important to register with a local GP as soon as you can. You can type in your home postcode on the NHS choices website www.nhs.uk and find a list of the local surgeries, doctors and details of their opening times. Please be aware that every GP surgery serves a different area, please call NHS Hounslow on 0800 953 0676 for advice.

How to register with a GP

You first must select a local surgery and call them to ask to register. You will need to bring with you a form of identification and proof of address, and you then will be asked to fill in a form. New patients will normally have a general check-up before seeing a doctor. We recommend that you do not wait until you are ill to do this.

Do you want to change your GP?

You can change your GP at any time. You do not have to give a reason for wanting to change your GP. The new practice may decline your request if you live outside their practice area.

• calling us on free phone 0800 953 0676 (from 09.00-17.00 Monday to Friday, you can leave messages outside these hours)
• email us at pals@hounslowpct.nhs.uk
• see them at Heart of Hounslow every Monday morning from 10.00 to 12.00  (they have a stand near reception)
