Adult Services
| Unscheduled and Home-based Services |
Unscheduled and home-based services are designed for people aged 16 and over who need nursing or therapy care within a community setting. |
| These are mainly for people who are unable to get to clinics due to their poor health, or immobility and include district nursing, community matrons, community rehabilitation, continence services, tissue viability services, treatment rooms and leg ulcer clinics, continuing care and phlebotomy services. |
| Most of these services can be accessed by referral via GP, another health professional, or from social services. |
District Nursing |
Patients who are house-bound and unable to attend a clinic or treatment room for their care can receive nursing care at home from the District Nursing team. The service provides wound care, general nursing care and palliative care, provided by skilled nurses and healthcare assistants. |
Referral via a GP, hospital, social services, or occasionally via self referral. |
Treatment Rooms and Leg Ulcer Clinics |
| These are clinics run by district nurses for patients with leg ulcers, or other nursing needs, and are designed for patients who are able to get to a nearby health centre. |

Community Rehabilitation Service |
Physiotherapists, occupational therapists and rehabilitation assistants provide rehabilitation and neuro-rehabilitation for mainly house-bound patients, who have lost their ability to be totally independent. |
| The service also includes the specialist nurse for multiple sclerosis, and also some clinic based services, such as the mobility clinic. |
Referral to the service is via a GP, other healthcare professionals, or social workers. |
Community Matrons |
Highly skilled nurses support patients with very complex, and multiple health and social care needs, both in their own homes and in nursing homes. They aim to prevent patients from unnecessary admission to hospital. |
Referral is via a GP, other healthcare professional or social worker. |
Primary Care Stream A&E |
| The primary care stream assesses and treats all walk-in patients (over one year of age) who attend A&E, but who could be more suitably treated by primary care practitioners. After assessment patients are directed either for treatment within primary care, or A&E departments, as appropriate. |
| The Primary Care Stream is staffed by senior nurses, nurse practitioners, extended scope physiotherapists, GPs and administrative staff. It operates 12 hours per day, seven days per week. |
| The scheme also educates patients regarding how to access appropriate services in primary care and helps patients who are not currently registered with a GP, to register with a local practice. |
Continence Service |
| The continence team provide expert advice, treatment and the provision of suitable continence products for children, and adults with urinary and bowel continence problems. |
Referral is via a GP. |
Tissue Viability |
| Patients with complex wounds are treated in a variety of settings: community clinics, hospital outpatient clinics, and as hospital inpatients. The service is managed by a tissue viability nurse consultant, using the latest concepts and techniques. |
Referral is via a GP (except in a hospital setting, where referral is via a consultant). |
Continuing Care Team |
Patients applying for ‘continuing care funding’ are looked after by the continuing care team. This team of nurses provide expert assessments, and also provide general nursing care to all residential homes across the borough. |
Referral is via a GP, or residential home. |
Phlebotomy Services (blood testing) |
For people needing blood tests there are many facilities across the borough where this can be done. Your GP will give you a referral form and you can make an appointment at your own convenience by calling the central booking service on the freephone number included on the form. |
For house-bound patients known to the district nursing service, the phlebotomist can take a blood test in the home following a referral from a GP. |
